Hydrochlorothiazide is a diuretic, low cost, which is in the rank of the first-line antihypertensive drugs in the world to be preventive treatment guidelines of hypertension, recommended for using alone or using as the drug combination in many types of hypertension drugs. Therefore, it is often used as the preferred antihypertensive drugs for elderly patients clinically. The mechanism of bringing high blood pressure down is to ease the retention of sodium and water and to reduce blood volume; at the same time it can reduce the burden on the heart, playing a role in the treatment of hypertensive heart failure. This is because the diuretic through increasing urine output makes the blood volume reduce within the body, reducing the blood flow from venous back to the heart, thus reducing the work burden on the heart.

However, the clinical findings show that the diuretics is a double-edged sword. It helps the treatment of coronary artery disease, meanwhile long-term use can lead to the concurrency of coronary heart disease for patitents with hypertension. If after long-term orally taking hydrochlorothiazide, it may cause the acid-base imbalance of blood, resulting in cardiac hypertrophy, the increase of oxygen consumption, inducement of coronary artery disease; it also induces the increase of low-density lipoprotein and the decrease of high-density lipoprotein level, therefore it will promote the hardening of the blood vessel to induce coronary artery disease.
